For Patients
You may get a short survey by text after a birth control visit. It helps us make your care better.

Why We Ask
Everyone should get good birth control advice and care, with no pressure. We work with Upstream, a group that helps health centers improve birth control care. Upstream asks patients about their visit to learn:
-
Did You Feel Heard?
How happy you were with the advice you got.
-
Did You Get the Full Picture?
If we talked about all of your choices.
-
Was It Your Choice?
If you felt free to pick the method that is right for you.
-
Did You Get It Fast?
If you could get your method the same day, and how you felt about the wait.
How the Survey Works
-
Sent by Text
About 1 day after your visit, you may get a text with a link. It comes from Upstream USA and asks about your last visit, including telehealth visits.
-
Short and Private
The survey takes about 5 to 10 minutes. You do not have to give your name, and you do not have to take it.
-
Get a Gift Card
If you finish the survey, you get a $20 e-gift card.
-
Many Languages
The survey comes in English, Spanish, Portuguese, Chinese, Vietnamese, Haitian Creole, Arabic, French, Lao, Somali, and Russian.
Who gets the survey? Patients ages 18 to 44 who were assigned female at birth and had a birth control visit. You will get it no more than once every 6 months.
Your privacy: Upstream never sees your health records. A partner called Azara sends the texts.
Don’t want texts? Reply STOP. If you don’t reply or take the survey, you may get up to 2 reminders.
